Our home visit practitioners are the same registered professionals who deliver care in our clinics: AHPRA-registered Physiotherapists, Occupational Therapists, Podiatrists, and Psychologists; ESSA-accredited Exercise Physiologists; SPA-certified Speech Pathologists; and Dietitians Australia-accredited Dietitians. Home visit services are available to NDIS participants, aged care clients under the Support at Home program, DVA cardholders, post-surgical patients, workers compensation and CTP claimants, and anyone who finds it difficult to attend a clinic in person. CAHC practitioners visit patients in their own homes across Greater Sydney & Queensland. Home visits are available for NDIS participants, aged care clients under the Support at Home program, DVA cardholders, post-surgical patients recovering from orthopaedic or neurological procedures, and any patient for whom travelling to a clinic is difficult or clinically inappropriate.

Aged Care Residential Facilities
CAHC provides on-site allied health services at aged care residential facilities — bringing expert clinical care directly to residents without the need for facility transport or external appointments. A regular visiting schedule can be arranged for residents across multiple disciplines, making it simple for facilities to coordinate care across their full resident population.
Facility managers are welcome to contact CAHC’s intake team to discuss an on-site allied health program tailored to their residents’ needs.
Schools and Childcare Centres
CAHC Speech Pathologists and Occupational Therapists provide assessment and therapy at schools and early childhood settings — bringing specialist paediatric services to where children already are. This is particularly valuable for NDIS-funded paediatric clients, where assessment in a familiar natural setting can produce more clinically meaningful results than a clinic-based appointment.
Community and Other Settings
CAHC practitioners can also deliver services in supported independent living (SIL) accommodation, community day programs, community centres, and workplaces — including ergonomic assessments and on-site WorkCover rehabilitation for injured workers.

Physiotherapy CAHC Physiotherapists deliver home-based assessment and treatment for musculoskeletal conditions, post-surgical rehabilitation, falls prevention, neurological rehabilitation, and mobility support. Home visit Physiotherapy is particularly valuable for patients recovering from orthopaedic surgery, older adults managing chronic pain or balance difficulties, and NDIS participants with mobility limitations that make clinic attendance difficult. Exercise Physiology ESSA-accredited Exercise Physiologists deliver supervised exercise programs and functional conditioning in the home environment — making use of the patient’s garden, backyard, lounge room, or available indoor space as the training setting. Home-based Exercise Physiology is particularly relevant for NDIS participants, aged care clients, and patients managing chronic conditions under the Support at Home program or Medicare CDM.
Occupational Therapy CAHC Occupational Therapists conduct home modification assessments — identifying and recommending changes that support safe and independent living in the patient’s actual home environment. They also deliver functional capacity assessments, assistive technology trials, and daily living therapy. Home-based OT is often the most clinically accurate form of OT assessment: the practitioner can directly observe the real environment, the real hazards, and the real daily living challenges. Speech Pathology CAHC Speech Pathologists deliver communication therapy and swallowing (dysphagia) assessments in the home — particularly important for post-stroke patients, aged care residents, and NDIS participants who benefit from assessment in their natural communication environment. Paediatric Speech Pathology is also available at home for NDIS-funded children, where familiar surroundings often produce more representative assessment results. SPA member.
Dietetics CAHC Accredited Practising Dietitians (APDs) provide nutritional assessment and dietary management in the home — relevant for aged care clients, post-hospitalisation patients, and NDIS participants with disability-related nutritional needs. A home-based appointment allows the Dietitian to observe the patient’s food environment, pantry, and meal preparation space directly, which often provides more useful clinical insight than a clinic consultation. Podiatry CAHC Podiatrists provide foot and lower limb care at home — including nail and skin care, diabetic foot monitoring, and orthotic-related review visits. Home visit Podiatry is particularly valuable for aged care clients, NDIS participants, and patients with mobility limitations for whom clinic attendance is difficult or where foot care needs to be delivered in a regular, accessible setting. What Happens During a Home Visit
Many patients book their first home visit without knowing exactly what to expect. Here is what the process looks like from booking to follow-up.
Before the visit: When you book a home visit, CAHC’s team will ask about your home environment — whether you have a suitable space for assessment and any exercise, whether you have mobility aids or equipment, and whether there are any access considerations (stairs, narrow doorways, parking for the practitioner). The practitioner will confirm the address, expected arrival time, and anything specific you should have ready before they arrive.
What to prepare: Most home visits require only a comfortable, clear space — a lounge room, bedroom, or outdoor area is sufficient. For Physiotherapy and Exercise Physiology appointments, wear comfortable clothing and footwear appropriate for movement. For Occupational Therapy home visits, the practitioner will want to see the areas of the home most relevant to your daily activities — typically the kitchen, bathroom, bedroom, and entrance. No special preparation is needed for Speech Pathology, Podiatry, or Dietetics home visits, though having any relevant referral letters or funding documentation ready is helpful.
During the visit: Your practitioner arrives at the agreed time with their own equipment — including portable assessment tools, exercise equipment where relevant, and any clinical supplies appropriate to the service. The session follows the same structure as a clinic appointment: assessment, treatment or therapy delivery, and a discussion of next steps. Visit duration is the same as a standard clinic appointment for the relevant discipline.
After the visit: Your practitioner will provide any home exercise program, home care advice, or written recommendations before leaving. Follow-up appointments can be booked during the visit or by contacting your nearest CAHC clinic directly.
You are welcome to have a family member, carer, or support person present during any home visit — and for many patients, particularly those in paediatric or aged care settings, their involvement is an important part of the therapy process.
Fees and Funding for Home Visits
Home visit services are available across all major funding streams. Travel fees are a specific consideration for home visits that do not apply to clinic appointments — the funding rules for travel vary by stream and are outlined below.
NDIS (National Disability Insurance Scheme)
Home visits are available to NDIS participants whose plans include support for in-home therapy under the Capacity Building — Improved Daily Living budget category. NDIS Pricing Arrangements allow for practitioner travel time and travel costs to be charged separately from the therapy session itself.
Eligibility for NDIS-funded home visits depends on your individual NDIS plan goals — speak with your support coordinator or contact us to confirm.
Support at Home Program (Aged Care)
Home visits are a core delivery channel for the Support at Home program, which replaced the legacy Home Care Packages framework on 1 July 2025. CAHC delivers allied health to eligible participants at home under Support at Home across Greater Sydney & Queensland.

DVA (Department of Veterans' Affairs)
DVA Gold Card holders are entitled to allied health home visits for all clinically necessary treatment — with no out-of-pocket cost for the therapy session itself. A D904 referral form (available from your GP or specialist) is required. Confirm current DVA travel policy with your GP or DVA.
Medicare Chronic Disease Management (CDM)
Eligible patients with a qualifying chronic condition can access up to 5 allied health sessions per calendar year under a GP-initiated CDM plan. Home visits can be delivered under CDM for eligible disciplines and eligible patients.
Important: the Medicare rebate applies to the clinical session only — travel fees for home visits are not rebated under Medicare CDM.
Rebate amounts are subject to change — confirm current rates with your GP or the clinic.
WorkCover NSW / SIRA
Home visit services are available for WorkCover patients where clinically indicated and subject to insurer approval. CAHC’s Free Insurance Claims Advisory handles insurer correspondence on your behalf — there is no administrative burden on you or your GP.
CTP (Motor Vehicle Accidents)
Home visit services are available under CTP claims within the 52-week statutory benefits period where clinically indicated. CAHC’s Free Insurance Claims Advisory manages CTP correspondence.
Private Health Insurance
Some private health fund Extras policies cover home visit allied health. Coverage and travel fee treatment vary significantly by fund and policy level. HICAPS on-the-spot claiming is available at point of service — bring your health fund card. Check with your fund about home visit coverage and any annual limits before booking.
Self-Funded / Out-of-Pocket
Home visits are available as a private appointment for self-funded patients. A travel fee may apply in addition to the standard session fee. Contact your nearest CAHC clinic for current pricing information.
Who Our Home Visit Practitioners Are
Every CAHC practitioner delivering home visit services holds their relevant professional registration or accreditation: AHPRA registration for Physiotherapy, Occupational Therapy, Podiatry, and Psychology; ESSA accreditation for Exercise Physiology; SPA membership for Speech Pathology; and Dietitians Australia APD status for Dietetics. All practitioners carry appropriate professional indemnity insurance, and all are CAHC employees or contracted practitioners subject to CAHC’s clinical quality oversight — not third-party referrals.
Before delivering services, all CAHC home visit practitioners conduct a brief safety assessment of the home environment as part of standard professional practice. If any aspect of the environment requires adaptation, the practitioner will discuss this with you before the session begins.
Multilingual practitioners are available for home visits across the network — Vietnamese, Mandarin, Cantonese, Korean, Norwegian, and Laotian speakers are available in various locations. For GPs and Medical Specialists
GPs can refer patients for home visit allied health via a CDM plan, a DVA D904 referral, a WorkCover or CTP claim, or a standard referral letter for private patients. Home visits are clinically appropriate for post-surgical patients, patients with complex presentations that limit safe mobility, elderly patients at elevated falls risk, and patients for whom transport barriers represent a genuine clinical obstacle to care. CAHC’s intake team will confirm home visit availability in the patient’s location before the first appointment is scheduled.
For NDIS Support Coordinators
CAHC provides home visit services for NDIS participants across NSW and QLD under service agreements. All CAHC NDIS home visits are delivered by registered or accredited practitioners with appropriate indemnity insurance. NDIS travel costs are charged in accordance with the NDIS Pricing Arrangements.
